Most accidents are due to a known threat that is addressed by regulations and company requirements, but where the safeguards have been allowed to degrade over time (technical, human or organizational). Hazard identification and processes for organizational learning have been shown to work, but during a major accident event, the barriers that were specified have not worked.

08:45 - 10:00: Plenary session - 'Dynamic Barrier Management'.

DNV GL will present the latest thinking in operationalizing existing barrier management practices and using operational data to actively manage barrier degradation. Dynamic barrier management aims to make use of the data-rich operational environment to provide feedback on barrier conditions and change in risk on a real-time basis.

Speaker:

Richard Palmer is head of DNV GL’s oil and gas operations in the Australia, PNG and NZ region. He is a naval architect with more than 17 years of experience in risk assessment and risk management of marine transportation and installation projects relating to offshore oil and gas projects worldwide, both from an analytical and an operational viewpoint. He has been involved in a number of marine transport accident investigations involving barrier failure.
